Medication plan
Reliable medication through more information
The Medication plan module provides the patients themselves and all attending physicians, pharmacists, care services and hospitals with an overview of all the patient’s medication and prescribed drugs.
The electronic medication plan supports healthcare professionals in choosing, prescribing and dispensing medication quickly, safely and without mistakes. Drug interactions are tested automatically, allowing the attending physicians to detect potential interactions and intolerances and avoid drugs that are critical for the patient. The risk of incorrect medication is also greatly reduced with the electronic medication plan.
For attending physicians
- Automatic checks help to avoid undesirable interactions between different medicines and to find the most appropriate choice of medication, avoiding unwanted side effects.
- The overview of current and previously prescribed medication increases patient safety.
- The electronic medication plan can be incorporated into information systems via a standardized interface and simplifies the preparation of prescriptions.
For chemists
- Legible prescriptions simplify the identification of medicine and active ingredient designations, also simplifying work in pharmacies.
- Thanks to the overview of all medication, the clarification burden is reduced.
- Multiple prescriptions or undesirable interactions between prescribed medicines are identified more quickly.
For carers and hospitals
- The overview of all medication helps carers in preparing pill dispensers for patients suffering from multiple conditions and reduces the risk of incorrect medication, thus improving the desired effectiveness.
- Upon admission, the hospital has an overview of all the patient’s medication.
- When discharging a patient, the hospital can consolidate the medication data more simply.
A patient’s medication plan is registered electronically by a healthcare professional once the patient has given his or her consent. The module then synchronizes all available medication data from different source systems with the medication plan. Any attending physicians granted access to the file can now view all the necessary information.
Swiss Post’s electronic medication plan is in use in the canton of Geneva.
